Breaking Taboos: Erectile Dysfunction

The WHO defines erectile dysfunction as the "inability of a man to get or maintain an erection." The problem begins when men start having problems during the sexual act. When they start with the encounter, many situations can happen: maybe they cannot achieve the appropriate stiffness to penetrate their partner or can penetrate but become flaccid too soon (whether they can ejaculate or not).  

This problem is divided into two crucial parts:

  1. The physical part, in which we need to evaluate the complete andrological state of our patient. This requires bloodwork and metabolic and anatomical studies that can be the cause for the dysfunction: from hypertension, diabetes, heart failure, kidney failure, immunological diseases, anatomical causes, or medical treatments. In any case, the intention is to verify the treatment to correct the dysfunction and return to normalcy.
  2. The psychological part. I want to focus a little more on these cases; these are the ones we call "performance stress" that can appear in young patients and now, because of the great access to information, causes a lot of disinformation.

 As more progress is made in the psychological analysis of "performance stress," it is found that it is more common for it to occur in heterosexual couples. It is rare in homosexual couples.

While considering the psychological aspect, it is normal that the partner understands the first time; nevertheless, if it happens continuously in many cases, demands, jealousy, or fear substitute this comprehension. This will only worsen the problem, which will cause self-medication or distancing from the partner, which in the long term, generates distrust and performance stress, perpetuating a continuous cycle of inability to perform. Often, erectile dysfunction is improved by proper psychological treatment and can be overcome through appropriate communication.  

When the situation begins to present itself, it is advisable to seek a consultation with the urologist who, after evaluating your case and circumstances, can bring diagnosis and proper treatment. Within the techniques to apply, we have penile neurovascular rehabilitation; this therapy uses medication that increases vascular flow with an increase in tissue oxygenation that improves the quality of the erection.
